For Lead Agencies

Only when we receive the service agreement in our MNITs mailbox will we contact the person for consultation services. Please do not send us emails with names and phone numbers.

To begin our Consultation Services, we must receive a Service Agreement in our MNITs Mailbox from the Case Manager/County Representative/ Lead Agency to our UMPI Number – A639173000 with Service Code T1023 for 6 units.
Once we receive the Service Agreement we will review the person’s contact information in MNChoice and reach out to coordinate times for completing the Service Delivery Plan. After the process has been finalized, the completed plan will be uploaded to the MNChoice Portal, then we will notify the designated Case Manager/County Representative/Lead Agency listed on the Service Agreement for review and approval.
